Thank you for your order. Your order will be processed according to the ProBuild Wireless Device Policy.
Ordering New Service: Eligible associates - after your form has been submitted your phone will arrive in 7 to 10 business days. Your device will be activated and ready for use.
Exceptions / Business Critical - any position not identified as eligible will need a signature from a Senior Vice President level or above to acquire service.
If you choose an approved phone that is not “free” to the company, the expense of the phone will be deducted from your paycheck (see Wireless Add / Change / Delete form in Appendix B for costs.)
Transferring Service: After submitting your form service will be transferred in 3 to 5 business days. You will also be able to keep your current phone number.
Ordering Replacements: After submitting your form for replacement the device will arrive in 2 to 3 business days. We will expedite to improve delivery.
Changing Existing Service: If you chose to keep your current phone and it is an approved device please note this on your form and your service will be active in 3 to 5 business days. If you will be receiving a new phone then it will arrive in 7 to 10 business days. Your device will be activated and ready for use.
Deleting Service: If service needs disconnected please turn in the form and annotate the associate name and phone number of the device to be disconnected. Please send any unused hardware to:
